Payment, Cancellation, and No-Show Policy

Laibon Health & Wellness is committed to providing timely, personalized care and respectful scheduling for all patients.

Payment

Payment is due at the time of service unless other arrangements have been made in advance. We accept:

  • Cash-pay

  • HSA/FSA cards for eligible services

  • Select insurance plans, as listed

Patients are responsible for any applicable copays, deductibles, coinsurance, non-covered services, and outstanding balances.

Insurance

Insurance coverage is not guaranteed by the Practice. Patients are responsible for verifying participation, benefits, and coverage limitations with their insurance plan.

Cancellation Policy

We request at least [24 hours] notice for appointment cancellations or rescheduling whenever possible.

Late Cancellation / No-Show

Appointments canceled with insufficient notice or missed without notice may be subject to a late cancellation or no-show fee of $50.

Late Arrivals

Patients arriving more than 15 minutes late may need to reschedule if there is not enough time remaining for a safe and complete visit.

Forms and Administrative Requests

Certain forms, letters, medical clearances, or administrative requests may require separate fees and processing times.
